We hold regular stakeholder meetings with you, your family, and your support team. These meetings are vital to ensure everyone is working together with a shared understanding of your goals.

Your step by step on boarding
1. Our first chat
We listen to your needs, goals, and plan either in person, via your COS or your approved delegate.
2. Intake meeting
Team reviews documents (BSP, OT Reports, Meal time management plans, funding details including plan etc.) which with consent your COS will provide to us.
3. Intake supporting documents
We create a comprehensive matching document for review by you, your COS, your Guardian or plan nominee.
4. Visit us
You, your COS, your delegate or Allied Health Providers can visit the home to ensure it has everything you need.
5. Risk and support plan
We plan for your safety and individual support.
6. Plan and agreement
We create a clear Service Agreement and transition plan which can be reviewed by your current team to ensure placement suitability.
7. Your team
Your allied health providers will help train and skill up your team with your specific needs.
8. Medical Care Team Member
Your GP, or relevant medical Care Team Member will complete our medication documents (if you have medications) so we can safely help support your health and wellbeing.
9. Start your service
Your supports begin as planned in our agreement.
10. Review and check In
We regularly check in to ensure you’re happy.
11. Communication
Commitment to you and your stakeholders team with regular stakeholder engagement and meetings.
12. On going support
With your consent we will provide weekly summaries to your stakeholder team for 3 months, and then monthly from then on.
